We ask that you arrive 30 minutes prior to your scheduled appointment time to complete the new patient paperwork. Make sure to bring your driver’s license and insurance card to your appointment.

To help expedite the paperwork process, bringing a list of your medications, previous surgeries, and past medical history is helpful.

Our providers must work by appointment. Occasionally, patient appointments do require longer time frames than scheduled. When this happens, your patience is appreciated. A sincere attempt is made to adhere to the clinic schedule as much as possible. The registration staff will advise you if the provider is significantly behind schedule. We ask that whenever possible, a return appointment for regular checkups be made before leaving the clinic.

We understand that scheduling conflicts occur from time to time. However, we request 24-hours advance notice if you are unable to keep your scheduled appointment(s). Two or more missed appointments may result in you being dismissed from Faculty Physicians Knoxville, PLLC. A $25 fee may be incurred after the second missed appointment for not providing the office with prior notice of cancellation.

We ask that you call our office during regular office hours to discuss rescheduling the appointment. Please keep in mind that this will appear as a ‘No Show’ appointment unless notification occurs within 24 hours prior to your appointment. Recurrent appointment ‘No Shows’ could result in your dismissal from our practice.

Faculty Physicians Knoxville, PLLC. is dedicated to serving our patients 24 hours a day, 7 days a week. The most effective way to serve you is during regular clinic hours, but we understand acute illnesses can occur at any time. After office hours and on weekends, our telephone is answered by our answering service, which relays the message promptly to the provider on call for emergencies. This may not be your usual provider, so be prepared to answer some health history questions for the on-call provider if needed.

If you feel your condition is truly a critical emergency, please call 911.

Please use the emergency room only in a true emergency (i.e. chest pain, shortness of breath, stroke-like symptoms).

To avoid delays and busy phone lines, the best time to obtain your medication refills is at your office visit. While we realize there may be a need to request a refill via telephone or patient portal, please allow at least 72 hours for all refill requests before checking with your pharmacy.

Sample medication will only be distributed during normal business hours.

Monthly refills of any controlled medications (pain medication, anxiety, etc.) will only be given during an office visit within regular business hours.

When calling the office for the prescription refill, please follow the prompts to choose your provider’s nurse line. Please leave the following information: your name, date of birth, Faculty Physicians Knoxville provider, medication name and dose, and pharmacy name and number. You may also use Athena Portal to communicate with your provider and their staff regarding the refill.

If a simple form is completed during an office visit there is no charge. Examples of a simple forms are handicap tab/sticker, concussion clearance, WIC, Home Bound Status Short form, college, and camp forms. If a simple form is not filled out during the office visit, the fee is $5/form. If you have not been seen regarding the health conditions associated with the simple form, you must be seen by your provider prior to the completion of the form.

Complex forms will not be completed during an office visit. Please allow 10 business days for your provider to complete these forms. Examples of complex forms include Short Term Disability forms, Long Term Disability forms, and FMLA paperwork. The fee is $25/form.

Fees are to be paid when a form is picked up.

Yes, you will be provided with a Good Faith Estimate that will estimate the maximum cost for your visit.

The estimate must be provided within 3 days of a request from the patient, within 1 business day of scheduled services to be provided in 3 business days, and within 3 business days of scheduling services to be provided in 10 business days.

This only applies if an appointment is made at least 3 days in advance.

Yes, you will be given an out-of-network charge that will be your responsibility for seeing our provider. The Out of network charge is incurred by your insurance company for not seeing a provider in your carrier’s network. Please ask your insurance company for details before making an appointment with our practice.
